If you are pregnant or have a newborn up to six months old and need assistance to provide basic items for your baby, contact your caseworker or a social service provider to find out how the Baby’s Bounty program can help you.
A request can ONLY be made by a CASEWORKER or SOCIAL SERVICE PROVIDER through the referral process. The items must be picked up by a CASEWORKER, by appointment, from the Baby’s Bounty™ distribution center.

How to use the Referral Form:
CASEWORKERS Please fill out the form completely. Fax or email when the mother has given birth. We will contact you to schedule a pick up appointment when the items are ready for your client. A typical distribution includes a portable crib, car seat, bathtub and toiletries, front carrier and a collection of seasonal sleepers, outfits, hats, socks, bibs, burp cloths, bottles, blankets, toiletries, diapers, wipes, a hand knit blanket and an infant toy.
For First-Time Referrals:
If your agency has never referred a client to Baby’s Bounty before, please have an administrator call our office and speak to the Director about becoming a referring agency.
